Celebration of Life
During this year's 4-H Volunteer Conference, we will honor those in our 4-H communities who volunteered and supported 4-H volunteers. Our Celebration of Life will be part of the program and pay tribute to those who have passed away since October 1, 2025.
Please complete this form before September 15 for inclusion in this year's celebration of life. Be sure to include a photo of the honoree. We have included space for two so that you may include a head shot and an "action" shot if you like.
