Mentor High School Sports Hall of Fame Nomination Form
All Individual and Team Nominees must have graduated seven (7) years prior to the nomination.  Coach nominees must be retired and have coached in the Mentor Schools for 6 years.  Please be as complete as possible.  Incomplete applications will be returned to the nominator for completion.  Please feel free to add more information via email to connors@mentorschools.org Nomination forms will be kept on file for a period of 5 years from date of receipt.

Fall of 1971 Soccer Team, started from nothing. First year ! We played against anyone who would take the match. Finished the year 9 - 1 - 1 record. And so, the soccer program was started by US.
