I understand that participation in baseball may result in serious injury and that protective equipment does not prevent all injuries to players. I hereby release from any and all liability, and agree to hold harmless, any and all individuals or entities associated with or involved in, Palatine Baseball and Softball including but not limited to the organizers, Board of Directors, managers, coaches, sponsors, supervisors and other volunteer and any paid participants. Also included in this release are any and all hosting locations, facilities or entities affiliated with any baseball activities. I knowingly and voluntarily waive any claim against the aforementioned that may arise out of any accident, injury, illness or death to my child that may occur during her participation in baseball activities including, but not limited to tryouts, games, practices, meetings, team parties, ceremonies, travel and tournaments, whether caused by negligence or any other cause. By signing, the parent/guardian agrees that Palatine Travelers, The Village of Palatine, Palatine Park District, Palatine Community Baseball Softball, and anyone else associated with this program shall not be held liable for any accidents, medical, dental, or other injuries sustained from participating in the program. I shall indemnify, defend and hold harmless Palatine Travelers, its agents and anyone else associated with this tryout from and against any and all claims, demands, suits, judgments, losses or expenses of any nature whatsoever (including, without limitation, attorneys’ fees, costs and disbursements), arising from or out of, or relating to any injury. I certify my child is in good physical condition and able to participate in any physical baseball related Activity. I certify that the information contained on my childʼs tryout waiver form is accurate. I understand there is an additional cost to this program that will be determined by the team manager. I understand that if my son is offered a position, a partial payment will be paid directly to the team within 24 hours of accepting an offer. I understand that the travel team committee makes the final decisions on who makes the Travel Teams based on ratings generated by qualified coaches of the travel program and previous known playing experience and behavior. *
