I give my student permission to attend the CHS Project Graduation trip to Six Flags Discovery Kingdom in Vallejo, CA. Students will leave on graduation night, June 5 and will return late on June 6. I give permission for his/her bag(s) to be searched prior to boarding the bus. I understand that participation in this activity is entirely voluntary and requires participants to abide by CHS standards of conduct. I release CHS Project Graduation and all of its' volunteers, related parties, or other organizations associated with activity from and all claims or liability arising out of this participation. I also understand that if my child needs to be removed from participation on the trip for violation of any rules, it is my responsibility to see that he/she is transported home. In the event of illness of accident, I grant permission to those persons in charge to take the steps for the proper treatment and care of my child. *

I have read and agree to follow these standards of conduct during the trip: 1. The possession or use of tobacco or vaping in any form on a Crater Sponsored trip is prohibited at all times. 2. The consumption, possession or sale of any alcoholic beverage at any school sponsored activity is prohibited (ORS 471 410). This includes consuming alcoholic beverages before or during the trip at any time. Violations will be referred to the proper law enforcement agencies and parents will be notified. 3. The unlawful possession, use, sale, or supply of any narcotics or dangerous drug at any school sponsored activity is prohibited (Chapter 743, Oregon Laws of 1971, Article 31). Violations will be referred to the proper law enforcement agencies and parents will be notified. 4. The possession of paraphernalia used for/or that could reasonably be construed to be used for the use of drugs is prohibited. 5. Valuable Personal Property: Please do not bring large sums of money or other valuable items. 6. Self Medication: Students in grades (9-12) are permitted to self-medicate prescription and non-prescription medication with parent permission.
