Covid contact tracing form
We are required by the State of Vermont to collect your contact information for contact tracing purposes each time you visit us. This information is only kept for 30 days. We will not share your information with anyone but the VT Dept. of Health and only if there is a need for contact tracing.

We are required to ask you to attest that you have followed the state requirements for quarantine prior to your visit, and that you understand that failure to comply with the state’s travel and quarantine policy may result in the loss of future skiing and snowshoeing privileges.

Please fill out a separate form for each household. Families traveling together can fill out one form for each family. We know this is a bit of a pain, so we thank you for helping us all do what we need to do.

In order to visit Vermont you must attest that all members of your party are feeling well, do not have a temperature and have followed the quarantine requirements below.   I certify that I have reviewed the State of Vermont out-of-state traveler guidelines and travel map and will comply with current health and safety requirements for traveling to, from, and within the State of Vermont. Failure to comply with the traveler guidelines may result in the loss of future skiing privileges. I further certify that I understand all travelers should stay home if ill (with any symptoms); maintain physical distance of at least 6’ from anyone outside their household; wear a cloth mask when in public spaces; and wash or sanitize hands often.
