This form is used for reporting incidents to East Ridge Athletic Association (ERAA). This form can be used for code of conduct infractions, unremedied issues and injuries (an injury report must also be filled out with your individual sport).
ERAA has a strict code of conduct in place to ensure the safety and wellbeing of all players, coaches, officials, spectators, parents/guardians and board members. Refer to the ERAA Operating Manual located on

ERAA's goal is to manage incident reports with professionalism, fairness, respect, thoroughness, confidentiality and consistency. We will treat all parties involved with respect and make sure their voices are heard without bias. ERAA has the final say when it comes to incident reports needing action.
*** Your report will be confidential (pertaining to code of conduct reports) unless there are matters that need legal intervention and/or under special circumstances it is vital identification is used. There will be prior notification of identity reveal if action is needed. Only the ERAA Executive Board will have access to incident reports. All incident reports will be formally documented per the incident reporting protocol.
