Please complete the following form to sign up for the Safe Plate Course. Registration of this form does not secure a space for the session until registration fee is paid. Registration is $140 for books, exams, and any additional educational materials.
Classes are on Monday August 22nd and Monday August 29th from 8:00am till 5pm with Review and Exam on August 29th.
Location of the class will be held at the Alexander County Service Center at 151 West Main Ave, Taylorsville, NC 28681
