Description : This form is for the purpose of requesting a booster for an event and must be submitted by the cadet in charge or assistant cadet in charge AT LEAST 2 weeks prior to the event. The cadet in charge or assistant cadet in charge must complete this form as a part of their CIC checklist and must fill out with the corresponding information in order to have a booster(s) assigned to their event. All information must be accurate, current, and must match with the information from the POC request form. If this form is not completed, boosters possibly may not be assigned to the event. DO NOT WAIT UNTIL LAST MINUTE. Get booster request form as soon as you have gotten request for your event.

ONLY FILL OUT IF 15+ CADETS ARE REQUESTED

Plans Squadron Commander, Vice Commander, or Superintendent will be in contact with the CIC to confirm boosters once request form is submitted and processed.
