2022-2023 Parochial School Transportation Agreement Form
By submitting this form, I am obligating my parochial school student to ride a Lebanon City Schools school bus for primary transportation to an/or from school.  I understand that failure to consistently ride the school bus without communicating with the Transportation Department will result in my student being removed from the route.

To ensure your student is routed on a bus for the first day of school in August, this form MUST be completed by 6/3/2022. Please submit one form per student.

The district understands that circumstances may arise when a student will not be riding the bus temporarily  (like illness or extracurricular activities), please notify our office if this occurs to avoid being removed from the route.
