2019 - 2020 Parochial School Transportation Agreement Form
By submitting this form, I am obligating my parochial school student to ride a Lebanon City Schools school bus for primary transportation to an/or from school. I understand that failure to consistently ride the school bus without communicating with the Transportation Department will result in termination of further service.

Please submit one form per student by July 1, 2019, in order to be placed on a route for the first day of school.

The district understands that circumstances may arise when a student will not be riding the bus (like illness or extracurricular activities), but most days your child will be expected to ride the school bus.
