I am prepared to make a 10-15 year commitment

I will accept and treat the condition of heartworms if the animal tests positive

I will not tie up or chain my pet and leave unattended outside alone

If, for any reason, I am unable or unwilling to keep this pet, I agree to return the pet to the original pet owner. I will not give this dog away, take to a shelter, place in another home(family and friends included) or sell this pet without written consent from the original pet owner

I am financially able to provide routine and emergency care for this pet for their lifetime. That includes but is not limited to food, boarding(if necessary), regular vet care, heartworm prevention and flea/tick prevention

If a spay/neuter is not completed before the pet is in my care I agree to complete the surgery at my own cost within 30 days from the date on this application

I am responsible for any and all actions of this pet while in my care

I understand that if I am not in compliance with this agreement at any time, the original pet owner holds every right to take back possession of this pet.
