Use this form to nominate event/location combinations that deserve recognition for their pivotal role in the history of pharmacy and pharmaceuticals across North America. 

Eligibility: Key events of national or statewide influence or meaning in history of pharmaceutical discovery, medication use, pharmacy practice, pharmacy education, pharmacy associations, and related activities. Examples:          

o Discovery or first synthesis of first-in-class pharmaceuticals

o Locations of pivotal events in science or clinical practice.

o First pharmacy classes at schools (faculties) of pharmacy

o First meetings of pharmacy associations and regulatory boards

Not eligible: First pharmacy in a town [but eligible if... first or oldest pharmacy in a state]. Pharmacies cited as part of historic business districts without qualifying distinction. Living persons or persons deceased < 20 years.

3. Series of questions about location of the event ... based on building where event occurred ... where key person worked (desk, lab)?

3f. Other location info to reduce ambiguity or aid recognition (especially if street address is not adequately descriptive):  Bldg name or #, intersection, crossroads, etc. 
4. Extended Description (When / who / what / how / why significant). Target: about 250 words or 1,200 characters (range 800-1,500 characters). Hard cap: 2,000 characters (with spaces). Yes, with spaces! Give original names, then explain and date any renaming. Include terms or acronyms important when users search the dataset. No need to spell out acronyms that have their own map pins (editor will assist). 
If a physical marker exists, enter text exactly as shown on marker. Convert ALL CAPS to Sentence case.  See available templates.

5. Rationale. Explain why this event/location deserves to be selected (max 250 words) *
6. Support Material

6a. List URL from website with substantive detail of the history of the event or organization. If you don't have one, we encourage you to add a detailed history-of-us page! That will be a good location for content that might not fit on the map pin. 
6b. List relevant URLs (website addresses) that provide additional written or graphic details about the event/location. Max = 3 URLs.
6c. List relevant journal articles or publications (with URLs if available). that provide additional written or graphic details about the event/location. Max = 2 publications.
7. Does a physical marker exist at this site? If so, list marker type (e.g., marker on pole), marker sponsor (e.g., Acme Historical Society), and date of marker (e.g., YYYY MM DD).
8. Presuming selection, describe how you would plan to announce selection (e.g., draft text for newsletter or press release). Describe what you would do locally to raise awareness about this project.   *
