Faculty Assignment Request Form
The Career and Professional Development Center (CPDC) offers a variety of career-oriented services that may be valuable to/complement your curriculum. To make our services more readily available to your students, please note that students can utilize Drop-In Resume Review/Quick Question hours, which are held every Monday and Tuesday from 11:00 a.m. until 1:00 p.m., every Tuesday and Wednesday throughout the semester from 4:00 - 6:30 p.m., or they may request an appointment. If you require or offer credit to your students to create/update their resumes or visit the CPDC for any of our services, please let us know by completing the form below so we can staff accordingly. Please submit AT LEAST four weeks in advance of your proposed class assignment deadline date. If you’re unable to do this, please reach out to Kelsey Thompson at
to determine if we can accommodate your request in a shorter time frame.
Please note: We ask that you please encourage your students to submit any documents that need reviewed by our office well in advance of their assignment due date. We will do our best to accommodate the high volume of requests we receive; however, if students wait to request our services until a few days before their assignment is due, we cannot guarantee we will be able to facilitate all requests in a time frame that meets the faculty member’s assigned due date.

2. All requests must be received a minimum of two (2) weeks prior to the requested date of the program and are granted on a first-come, first-served basis.
3. For multiple class assignments/requirements, please submit a separate form for each.
