Students will be routed both ways to and from home. Parents may communicate their schedules with the bus driver or school when this service will not be used. There is an option for alternate pick up or drop off locations at the bottom of this form.

Walker - You live in the "walk area" to the school. No bus service is available for those in the walk zone.

O/A - You live out of the attendance area. Bus service is not available from your home address. You can be assigned to the closest stop within the district.

Special Education Transportation. My student has an IEP with a transportation component.

A stop that is not used may become a "dead stop" with the driver no longer stopping.

I understand that my student will be routed to ride on a bus to and from Mounds View Public Schools. I understand that It is my responsibility to have my student ready to ride daily 5 minutes before the stop time or communicate with district transportation, bus company, bus driver, or school (pm) regarding an alternate schedule.
