Woodbridge HS 2017-2018 Club Application
Due on Friday 9/15 by the end of lunch. You must ALSO submit the paper application, constitution, and budget in addition to this form, in order to finish your application to be a club. Every previously existing club must re-charter every year.

Please complete this form accurately and do not submit multiple forms. This form should be completed by the President. Only one person per position is allowed. Each club must have one President, one Vice President, one Secretary, and one Treasurer. For this application, you are only required to fill out information for one President and one Vice President, if you already have your Secretary and Treasurer positions figured out, you may fill those sections out too. These four people may appoint additional people to their club cabinet as needed (i.e. Publicity Commissioner, President Assistant, e.t.c.).

This application will be invalidated if multiple people are listed for one position (NO CO-PRESIDENTS), or if the above rules are not followed. Please do not put "N/A" for required fields as this will invalidate the application.

Important: THERE IS A MANDATORY MEETING FOR CLUB PRESIDENTS ON SEPTEMBER 26 in the theater during lunch! You must attend or your club will be issued a strike.

Please Note:

1) You still must submit the packet consisting of the Constitution, Signature Page, and Budget

2) On all fliers or posters advertising your club, you must include the following phrase in order to insure that WHS is following the IUSD Non-Discrimination Policy:
