I acknowledge:

I understand that to rent a costume, a security deposit of $200 is required.  Upon returning the costume, you  will receive a refund of $180.00. Please note that a non-refundable rental fee of $20.00 will be retained.

Wici the end of year concert in May ,2027 (data will be provided at a later time).
I acknowledge:

a) Dancers are placed into a group according to age and capability. Any changes must be discussed with teacher/instructor.

b) Due to unforeseen circumstances the instructor/teacher may reserve the right to cancel practice or be substituted by another qualified instructor. The instructor will inform the parents/guardians of any changes.
c) Additional practices may be required before important performances.
d) Dancers must be present and be prepared for lessons on time.
e) Dancers cannot leave the classroom before the end of class. If necessary, students must request permission
f) Cellular phones must be put away during practice.
g) Chewing gum, eating or drinking beverages besides water during lessons is not allowed.
h) All members must be respectful of other students, instructors, parents and staff.
i) Attendance is important therefore, please inform the dance instructor of the dancer’s absence.
j) Instructor reserves the right to remove dancer from dance if he/she misses too many practices.
k) Not showing up to a performance is not tolerated
l) Only dancers are allowed at practice.
m) Running in the hallways is not permitted, nor is inappropriate behavior such as yelling.
n) Persons responsible for damages made to the facilities from which Wici is leasing will be the financial responsibly

a) All Małe Wici students will receive traditional costume from their teacher, and a security deposit of $200.00
per dancer must be provided to Wici.
b) Maintenance and processing fees of $20.00 will be deducted from the Security Deposit. However, dancers
are expected and responsible for polishing shoes, washing or dry-cleaning, ironing, and preparing the shirts
and/or undergarments for the costumes themselves. NEVER machine wash the traditional costumes without
receiving permission from your instructor. Only white shirts and undergarment can be machine washed!!!
c) Costumes must be returned the last day of practice. If the traditional costumes are not returned on time and/or
undergarments are not cleaned there may be a late fee applied.
*If costumes are damaged or lost charges will apply.
d) Some accessories for performances such as red ribbons, shoes, clogging shoes, hair pins, fake hair, etc…may be
needed and are not provided by Wici
e) Garment bags must be used to carry costumes. All dancers must have garment bags with the dancer’s name printed on bags.

All dancers must wear WICI shirt during practice, appropriate dance wear and dance shoes:

Girls - must wear Wici shirt or leotard, black character skirt, ballet or jazz shoes, black tights or/and
leggings. Hair must be pulled back (ponytail or bun)
Boys – must wear Wici shirt and black long pants, ballet or jazz shoes
Some groups will need clogging shoes (please ask your instructor)
WICI t-shirts will be provided

Requirements for parents:

Parents are not allowed in the practice room and hallways during class unless requested by the teacher. Parents should DROP-OFF child at the waiting room.
Parents/guardians should have the instructor’s telephone number.
Parents/guardians must provide all “direct” telephone number to the instructor.
Informational meetings are held at the end of every practice.
Parents should be attentive and watch their children before and after class.
We recommend that every WICI group have a Volunteer Parent Leader and/or Assistant Volunteer(s). The Volunteer’s responsibilities would consist of the following: maintaining a list of children, become a
representative for the parents and the group in order to relay information, and assist with record keeping (receipt and return) of the costumes Traditional Polish Folk Costumes owned by WICI.

Please inform teacher/instructor if your child has any allergy or any medical conditions.

I hereby authorize Wici Song & Dance to use photographs and/or video recordings of myself and/or my minor child(ren), including names and likenesses, for promotional, educational, website, social media, print publications, and training purposes.

Children ages 11 and up may eligible to travel abroad with their group to international dance festivals during the summer. Please ask your instructor for details.
