Liability Waiver and Release Form for any participation in the Physical Education (P.E.) class offered by CHEF of GNO.

P.E. coach, volunteer board members of CHEF of GNO, and volunteers or staff of First Baptist Church of Kenner specifically included as “Waived.”

I, (Name of Parent/Guardian on Form) hereby waive and release, promise to hold harmless and ceaselessly discharge CHEF of GNO, its coach, and volunteers of and from any and all claims, demands, causes of action or injury, lawsuits, damages, and liabilities, of all natures, whether it is known or unknown, in law or in equity, that I may have or ever have in the future while participating in any activities associated with the parties aforementioned in the agreement discharge. 

I understand that serious injuries, damage to personal property, and many other circumstances could occur while participating in any sport, activity, or movement while under CHEF of GNO supervision. On behalf of myself, my family, and representation, I waive all claims for damages done to myself or my property whether the incident is caused by an ordinary negligence of the released parties mentioned or otherwise.   

By clicking, “I Agree” I take full responsibility and waive all claims of personal injury, death, and damage to personal property while participating in any activities, functions, or work place opportunities associated with my participation (or my child’s), CHEF of GNO, its coach, volunteers, and volunteers or staff of First Baptist Church of Kenner. 

By clicking “I Agree” I recognize I am authorized to waive the right of responsibility from CHEF of GNO, its coach, volunteers, and volunteers or staff of First Baptist Church of Kenner for myself or listed “children” below. If one of the listed “children” is not my direct kin, I willingly take legal responsibility for any claims or personal injury, death, and damage to personal property while participating in any sport, activity, or movement that child’s legal guardian may bring about.
