By completing and submitting this form, I acknowledge that I (or my child) do not have any physical limitations, medical conditions, physical or mental disabilities that would limit or prevent me (or my child) from participating in a martial arts activity. I further acknowledge that I understand that capoeira is a contact martial art that is not without risk of injury. I, for my heirs, executors, administrators, assigns and personal representatives, release, waive and forever discharge Capoeira Males,  and its respective representatives, officers, directors, instructors, successors, and assign of and from all  claims (including all demands, damages, costs, expenses, actions and from all claim equity) in respect of death, injury, loss or damage to my (or my child's) person or property, however caused, arising or to arise by reason of my (or my child's) participation in the activities and/or services offered by Capoeira Males in any manner whatsoever, including but without limitation to any claims resulting from or arising out of the negligence of Capoeira Males and/or that of all the aforesaid or the negligence of any other person using or present on the premises used by you, whether they be members or non-members.

I further hereby undertake and hold harmless, and agree to indemnify Capoeira Males and all of the aforesaid from and against any and all liability incurred by Capoeira Males and any and all of them, as a result of or in any way connected to my(or my child's) participation in these activities and/or services offered by any means whatsoever.
