ITIN Application/Renewal Referral (Foundation Communities)
Please answer the questions to the best of your knowledge and let the client know we will contact them within TWO WEEKS (as of 3/15/23). In almost all cases, the client should have ALREADY HAD THEIR TAX RETURN PREPARED AND QR'D before you submit the referral.

Starting Monday, March 27th please provide the client with a completed extension form to mail in if they are not contacted and scheduled for the ITIN appointment on or before April 18th. If the client is applying for an ITIN, put "APPLIED FOR ITIN" in the space where you would have put the SSN/ITIN.
