7th Grade Scrooge! 2025 - Parent Chaperone Interest   
Hello Parents! I have 28 Parent Chaperone Spots open for the 7th Grade Scrooge! Theater Trip.

TRIP DATE:  December 12, 2025

Do not complete this form until AFTER you have registered your student on MyPaymentsPlus. 
If you do not register your child, they will not be allowed to go and you'll be taken out of the lottery.

SCHEDULE: (Subject to change)
8:45 am Parents Check in at Front Desk. Load Buses, take lunches
9:05 am ON BUSES, LEAVING UA (Approx. 35 min drive)
9:40 am Arrive/check-in, A Holiday Musical in 2 Acts, Approximate Run Time is 2.5 hours
10:00 AM SHOW TIME
12:45 pm Load Buses
1:05 pm Lunch at Stallings Park
2:05 pm Depart
2:40 pm Arrive at UA

This interest form closes on Nov 24th. please answer the questions below so that you may enter our lottery. Sadly not every parent who wants to go can due to capacity limits. Once the sign-up closes, you will get an email letting you know that you have been selected and how to register. Please use correct contact information - you will be notified from the information you provide here, not based on what we have on file in Infinite Campus. You may submit one form for each parent. 

All Parents must drive themselves to the Theater. Please feel free to car-pool. Parents, you may sign your child out with their teacher at the park if you'd like to take them home but only after all the students are back on the bus to return to UA. 
**If you are taking another student home in addition to your own, that child's parent MUST contact the teacher via email and give permission prior to the trip date. If that does not happen, the child will NOT be released to you. 

Please consider the following before filling out the form:
    * If you do not know for sure that you can commit to the day 100%, do not fill out form. 
    * Multiple entries do not increase your chances of being selected. Duplicates are deleted. 
    * Students are NOT ALLOWED to ride to the experience with you.
    * Siblings and other family members are NOT allowed to come along. This includes infants. 
    * Chaperones MUST stay the entire time UA is at the experience, you may not arrive late or leave early. 
    * If you have not paid your child's Yearly Activity Dues, you are not eligible to chaperone. 
    * Parents who are not chosen, please respect the process! If you show up to the experience and were not chosen to chaperone, you will not be allowed entry with our group. You will not receive Volunteer Hours and this behavior will disqualify you from participating in any chaperone lottery for the rest of the school year.

I understand that I must check in with Mrs. Susan Houchins at the front desk, park in the South Lot & head to the classroom to help teachers carry lunches to the bus. A visible UA Visitors Sticker is REQUIRED to attend. In some cases, we need parents to transport the lunch coolers.  *
I understand that I must sign my student out with the teacher before I can leave the experience with them.
