Camera Checkout Agreement Verification *
By checking the box below, you agree to the following terms of Mr. Doerr's camera checkout policy. Students are financially responsible for all camera equipment and are required to check out every piece of equipment used outside the school day. We understand that accidents happen and AT OUR DISCRETION we can waive any or all fees associated with loss, damage, theft or breakage of the Publication Department's camera equipment. By signing this form the student assumes this responsibility. Failure to bring equipment back or holding on to equipment longer than necessary will result in an obligation form being created with Akins High School. This obligation form will cause the student to be denied a report card and for seniors, the possibility of not being allowed to collect their diploma until the fee is paid. In addition, students who do not return equipment when requested will be subjected to a grade reduction of at least 10 points on their over-all grade for the 6-week grading period in which the incident took place. Students are responsible for checking out all equipment and its subsequent return in good order to either Mr. David Doerr or Ms. Laura Fleming. Any equipment that is not properly checked out can be assumed to be stolen and appropriate action will be taken with both Akins High School and the Austin ISD police department.
