NOTE: This release form is a contract with legal consequences. Please read it carefully before accepting it.
I want to participate in the 2023 season of Goody’s Ball Hockey activities. I understand and acknowledge that the sport of Ball Hockey may involve physical risk. I accept the responsibility for inspecting each establishment on which I play Ball Hockey in connection with Goody’s Ball Hockey and satisfying myself as to its safety. I also accept the responsibility for inspecting any and all equipment I intend to use in connection with the Goody’s Ball Hockey and satisfying myself as to its safety.

In consideration of Goody’s Ball Hockey accepting me as a participant during the 2023 season, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge Goody’s Ball Hockey, its sanctioning body and sponsors and all their respective agents, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns, of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death , injury, loss or damage to my participation in the 2023 season of Goody’s Ball Hockey, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2023 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

Furthermore, in accepting to participate in the 2023 season, I hereby acknowledge and accept that during this season, any and all promotional tools, including but not limited to pictures, videos and/or statements, can be used by Goody's Ball Hockey at their full discretion for any purpose they so deem fit.

I further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected with, my participation in the 2023 season of Goody’s ball hockey.

Any and all registration fees, as agreed upon by the parties, shall be disbursed at the time of registration or at any later date as directed by Goody's Ball Hockey. Any and all request for a refund after the original registration shall be at the full and final discretion of Goody's Ball Hockey and shall be evaluated on a case by case basis. Furthermore, if the registration fee is paid by personal cheque, please note that any deposit that is returned due to insufficient funds shall be subject to an administration and banking fee of $25.00. Said fee to be paid to Goody's Ball Hockey as soon as possible and shall be a stand alone and independent fee.

All players, companions, coaches, referees, staff and volunteers will need to follow the NB Provincial health Covid-19 protocol and the facilities operational plan.

By accepting this waiver, I acknowledge having read, understood and agreed to the above release, waiver and indemnity. I warrant that I am physically fit to participate in the 2023 season of Goody’s Ball Hockey.
