This Form is for the purpose of requesting travel subsidies for those attending the Local Organic Food Co-ops Network Annual Assembly to be held at Ignatius Jesuit Centre of Buelph, Feb. 23-24,2016.

Please note that this is a request form and that our ability to respond to your needs will depend on the volume of requests.  We have a limited budget for the Assembly, but find it very important to support as many food co-operatives to attend as possible, with priority given to those that travel long distances and to whom costs are most prohibitive.  We strive to be as inclusive as possible.

Requests will be processed as soon as possible and successful applicants will be informed on or before February 15, 2016.
