This agreement is entered into by the person completing this form ("Client") and SAFER Coaching & Consulting Services, LLC ("SAFER"), and pertains to coaching sessions which shall occur over the telephone, by video conference, or in person, as agreed upon between the Client and SAFER.  What is Coaching? The International Coaching Federation defines coaching as "partnering with clients in a thought-provoking and creative process that inspires them to maximize their personal and professional potential, which is particularly important in today's uncertain and complex environment. Coaches honor the client as the expert in his or her life and work and believe every client is creative, resourceful and whole." In each coaching session, you will choose the focus, and I will stand with you while you find your path forward. *

Fees: Payment is expected at time of service or in advance, unless prior arrangements have been made. *

Coaching Format & Sessions: We offer coaching sessions over the phone or through video conferencing; clients are expected to call SAFER or join the video call on time.  Please call from a quiet location, where you will be undisturbed for the duration of the session (preferably not from a moving vehicle). For local (Columbus, Ohio or Elkhart, Indiana areas) clients, we offer a limited amount of coaching sessions in person at a mutually agreed upon location, such as a local coffee shop, library, or park. *

Schedule Changes: Please give at least 24 hours notice if you need to cancel or reschedule an appointment. If you have an emergency, we will do our best to work around it with you. Otherwise, you may be charged for the session. *

The client understands and agrees to the following: I, the client, am employing SAFER Coaching & Consulting Services, LLC for the purpose of assisting me in structuring, identifying and achieving goals as they have experience in such matters and agrees to render such services. I am always free to accept or reject any observations, suggestions, or requests made by SAFER at any time. I authorize SAFER to be direct and unconditionally supportive of me throughout the coaching process. While there is no recognized coach-client privilege in law, all information shared with SAFER is kept confidential to the extent governed by the International Coach Federation's Code of Ethical Conduct. This code includes a prohibition against SAFER disclosing that I am a client, without permission. Occasionally certain topics and information may be shared anonymously with other coaching professionals for training, mentoring, or consulting purposes. Any details that could identify me as a client will never be disclosed without my express written permission. In the event of any dispute, claim, or grievance, by the client against SAFER, the sole remedy will be limited to the total fees paid to SAFER Coaching & Consulting Services, LLC. The client agrees SAFER is not responsible for any direct, indirect, or consequential damages beyond the fees paid by the client. The parties agree that the proper venue for a claim of any kind resulting from this coaching agreement shall reside solely in Franklin County, Ohio, USA. *
Duration of Agreement: This initial agreement is for 90 calendar days from the date of the first coaching session; our agreement will continue on a month to month basis, as negotiated between SAFER and the client. The client or SAFER may cancel at any time. *

Scheduled Coaching Sessions: The client understands and agrees to retain SAFER and meet via telephone or video conferencing (Zoom or Facebook Messenger), with the option of one of the following monthly rates:   *

I, the client, understand and agree to make coaching sessions a priority and to call on time for scheduled appointments. I further understand and agree that if I do not cancel an appointment 24 hours in advance, or call within 15 minutes of the scheduled session time, I will not be refunded for that session. Any uncompleted or unused sessions will not be carried over from one month to the next, except in the case of medical emergency. All fees are expected on the 1st of each month. As a coaching client, I understand and agree that I am fully responsible for my own well-being during my coaching sessions, including any choices and decisions I make. I am aware that I can choose to discontinue coaching at any time.
