Audio In Video Games - Survey Questionnaire
Dear Respondent,
 
I am inviting you participate in a research project to study the role of audio in video games. Along with this letter is a short questionnaire that asks a variety of questions about your experience with video games, in particular to do with audio and the concept of immersion.  I am asking you to look over the questionnaire and, if you choose to do so, complete and submit it. It should take you about 15 minutes to complete.

The results of this project will be used to support my secondary research on the role of audio in video games. Through your participation I hope to understand the effects that good and bad audio have on the gameplay experience. I hope that the results of the survey will be useful for supporting my argument that audio has the capacity to assist the player in establishing a deeper psychological connection to gameplay, and I hope to share my results by publishing them in a thesis.

I do not know of any risks to you if you decide to participate in this survey and I guarantee that your responses will not be identified with you personally. I promise not to share any information that identifies you with anyone. You will not have to put your name on the questionnaire. The survey is to be done online so there is no need to submit your responses in person.
I hope you will take the time to complete this questionnaire and submit it. Your participation is voluntary (and there is no penalty if you do not participate). Regardless of whether you choose to participate or not, please let me know if you would like a summary of my findings.

For example, the doorbell rings while you are completely immersed in playing a video game at home. You are completely unaware of what's happening in the real world and so you don't even hear the doorbell.
