Fingerprint/Safe Haven Request
This form is to be filled out if you have finished your LiveScan (fingerprint) back ground check and/or your Safe Haven online training within the Sacramento Diocese. These are requirements put forth by the Diocese to help ensure the safety of our students, faculty, staff and campus. The information you submit is completely confidential and will not be used for any other purpose than for acquiring your training and fingerprint clearance.
A few reminders:
1. Please fill out this form for INDIVIDUALS only. (For example: do not type "Smith family," but rather one submission for "John Smith" and another separate submission for "Jane Smith.")
2. Fingerprints in the Sacramento Diocese do not expire and should only be finished once. Date of birth is required by the Diocese to verify status.
3. Safe Haven online training expires every 3 years and needs to be up to date to volunteer on campus.
