Complete and Submit this form and I will be in touch via email to finalize details, answer any questions and send you am invoice for a $25 deposit to hold your date. This deposit is non-refundable and will go towards your final balance. Remaining balance is due upon pickup.
I do need ALL of this information, as accurate as possible, before being able to send your deposit invoice. As a small business, I like to understand as much as possible about my orders and therefore my time every week to accommodate as many clients as I can.
