Attend at least four regular member meetings in the coming year.

Pay the nonrefundable deposit of $50 by or on April 21 to reserve my spot in Rise Up Academy.

Pay for enrolled classes by the first day of classes.

Participate in at least one adult class to receive continued mentoring according to our bylaws.

I understand that if I do not attend four regular member meetings as described in this form and the Rise Up Academy bylaws, my membership status will be changed from "parent member" to "provisional member."

I understand that if I don't participate in at least one of the adult courses to receive mentoring according to the bylaws, then I am not considered a "current" member.

I understand that according to our bylaws, "current members seeking renewal shall be accepted prior to new applicants."

I understand that the board of directors reserves the right to decline membership or enrollment to any person for any reason.
