By initialing this form, you certify that you do NOT want your Federal Loans. Once nullified, the Entrance Counseling and Master Promissory Note (MPN) will no longer be required by the Department of Education.  This may take up to a week to reflect on your Award Letter.

In the event that you want your loans reinstated, you will need to submit a Loan Change form [located on InsideCBU] along with the completion of Entrance Counseling and the Master Promissory Note.

Any false statements or misrepresentations may be cause for denial, reduction, withdrawal, and/or repayment of financial aid and may be subject to fine, imprisonment or both, under provisions of the U.S. Criminal Code.
