Hold Request Form
I. There is a $25 fee to put your membership on hold. Upon reinstatement, this fee is credited towards your next payment. Please contact me directly at if you are holding your membership while on maternity leave or for an injury.

II. FIT4MOM Columbus North must receive the completed form 15 days prior to your next scheduled payment (i.e., if payment is due on the 27th, hold request must be received by the 12th).

III. You may request a membership hold for up to four months during any 12 month period.

IV. You will receive an email prior to processing the hold.

V. Membership payments automatically resume at the end of the hold period. We do not send notification prior to the payments resuming. It is imperative that you stay on top of your membership status and contact with any questions about the duration of your hold.

VI. To return prior to your hold period ending, email with your exact date of return to class. This will be the first day that your membership payments resume. It can be on any day of the month and the dues will be pro-rated.

VII. Subject to paragraph III, you may extend your hold period, by emailing 15 days prior to the end of your scheduled hold period.

VIII. To attend classes while your membership is on hold, you can pay the rate of $20/class. Please let your instructor know at sign in.
