The information is required in order to comply with the club’s safety, insurance and attendance record requirements. If we do not receive a completed form it may affect your ability to attend training and selection for match play. North East Cheshire CC cannot accept responsibility if any information is not shared. All data is held in accordance with the club's Privacy Notice.

If a player is in school Year 13 and below (or equivalent), they must complete the relevant U18 form rather than this Senior form. This can be requested from membership@neccc.co.uk
