Lyme sleep survey
I am a nutritionist researching sleep issues in Lyme disease. If you suffer (or have ever suffered) from Lyme disease and/or co-infections, I invite you to fill out this survey. If sleep issues were never an issue during your Lyme journey, please indicate by answering NO to the first question (in this case, you need not fill out the rest of the survey if you do not wish to; this will still help me in my research process).

The survey is completely anonymous. Thank you for your time. Sending healing thoughts your way!
-Kate Hope, MS, CNS

NOTE: The survey is written as if you currently have Lyme. If you do not currently have Lyme symptoms, please fill out the survey according to how you felt when you did have symptoms. Thank you!
