I hereby give permission for my child, to participate in the Medina County Police Activities League (MCPAL) program, and special events including field trips. I understand that my child will be involved in physical activity and I will need to supply MCPAL with medical information as it involves my child’s involvement with activities associated with MCPAL.

In consideration of the MCPAL program, granting my child permission to participate in MCPAL activities and field trips, the undersigned does hereby waive, release, save, and hold harmless and indemnify Montville Township, Medina City School District, Brunswick City School District, Cloverleaf Local School District, Highland Local School District, Wadsworth Local School District, Montville Twp. Police Department, Brunswick Police Department, Medina Police Department, Medina Twp. Police Department, Medina County Sheriff’s Department, Wadsworth Police Department and MCPAL, their organizers, officers, employees, volunteers, agents, and sponsors from any and all claims for damages to me or my child or personal injury to me or my child or loss or damage to property which may be caused by any act or failure to act on the part of the above mentioned entities and/or the MCPAL program, their organizers, officers, employees, volunteers, agents, and sponsors. The undersigned further assumes the risk of all dangerous conditions in and about the City of Medina, Montville Township, Brunswick City, Hinckley Township, Wadsworth City, Westfield Township and/or MCPAL property, both real and personal, and waives any and all specific notice of the existence of the dangerous condition, if any. I give my consent to use my child's name, comments, photograph and likeness in order to promote MCPAL. I understand that the student may be photographed or videotaped. I also certify that I am the legal guardian of the student with the right to give this consent.
