Incidents that warrant an Incident Report include serious injury, serious illness, any type of bullying, hazing, or grooming, any type of abuse (mental, emotional, physical, sexual), fighting, breaking the law, or any behaviors prohibited by the Blackhawks Code of Conduct.

The protocol for handling incidents is to attend to the needs of the individuals involved, notify emergency personnel when appropriate, document and communicate the details of the incident to the trip and club leader and contact the parents/guardians with the trip leader.

Upon submission, this report will be emailed to club personnel to address.
