As the player's guardian, by submitting this form, you understand and agree that:

Your application will be considered for the player’s inclusion in the Championships; however, acceptance or not of the player in the Championships or in a Qualification that may be held in advance of the Championships is at the sole discretion of the organizers, who are not obliged to specify the reason of possible non-acceptance.
Upon acceptance, you will be requested to pay the nominated entry fee at the nominated bank account of the organizers of the Event within 7 days, in order for the Acceptance to be secured. The entry fees are not refundable.
You consent that the organizers of the Championships will use the data provided in this form, and of potentially other such forms and documents that may follow, for the sole purpose of running the Event in the most efficient way following GDPR regulations.
