As a parent, I give my child permission to participate in the after school Strings Program. I will see that he/she practices at home and comes prepared for a lesson each week. I understand that I must provide an instrument (VIOLIN OR CELLO) and materials for my child to participate. 

Please note: Students do not need any prior experience for the Beginning Class!

Beginning Class Supplies needed are a music stand, a Kun shoulder rest or equivalent (for violin students only), and Learning the Violin, Book 1 by Cassia Harvey (for violin students) or Learning the Cello, Book 1 by Cassia Harvey (for cello students.) 

Intermediate/Advanced Class Supplies needed are a music stand and $10.95 for your lesson book.

Students who have never played an instrument or who are playing at a beginning level should sign up for the Beginning Violin/Cello Class. Please note: Students do not need any prior experience for the Beginning Class! 

Returning students or students who can read and play the notes in first position on the violin should sign up for the Intermediate Violin Class.

I understand that the tuition is $280 for the first 14 week session. I agree to pay full tuition on or before the first class, the week of September 22nd. Payment should be made by cash, check or money order made payable to Judith Harvey. Payment may also be mailed beforehand to Judy Harvey, 6403 N. 6th Street, Philadelphia, PA 19126  *
