The application for affiliate membership with Peoria NEXT Innovation Center must be completed in its entirety for prompt consideration.  Additional information may be requested as part of the application process.  All information submitted in or with this application may be subject to disclosure under the Freedom of Information Act except information designated as proprietary by the Applicant.  The submission of false or misleading information in this application will be grounds for disqualification from selection and/or dismissal from The Peoria NEXT Innovation Center.  This application will be reviewed within 30 days of its submission.


To be considered for affiliate status, the applicant must have a real need for the services offered.
