Refund Request Form/Enrollment Cancellation
  • Registration fee refunds can ONLY be requested by the account holder. 
  • This form must be submitted and received by 3 PM the day BEFORE the first league game for the season. FORMS RECEIVED AFTER THE STATED GUIDELINE WILL NOT BE ACCEPTED. 
  • Sports Programs may receive a partial refund if requested after uniforms/poms are purchased. 
  • CREDIT CARD CONVENIENCE FEES ARE NON-REFUNDABLE.  
  • PLEASE DO NOT CONTACT COORDINATORS FOR CANCELLATIONS/REFUNDS.  
  • PLEASE FILL OUT ONE FORM PER CHILD WHO YOU ARE REQUESTING A REFUND FOR. 
  • THERE WILL BE A $10 CANCELLATION/REFUND FEE FOR EACH PLAYER THAT CANCELS.
