Our new proposed transition pathway includes the opportunity for you and your young person to meet the adult specialist nurse at one of their paediatric appointments, and the adult specialist doctor, nurse and other relevant professionals at another paediatric appointment, so they can become familiar with who they will be seeing when transferred to the adult service. Do you think this would help/have helped the transition process for you and your young person?
