Please list all courses that you are willing and eligible to tutor, with list the corresponding letter grade you received in that course (e.g. ENG 1430 - A+). Note: a minimum grade of A is required for eligibility to tutor in a course.

Eligibility & Ethics Agreements *

By typing my full name below I am electronically signing this document and confirming the following. (1) I am hereby allowing UMES to add me to the UMES Tutor Registry until the end of the current school year. (2) I am aware that UMES is not responsible for tutor payments or scheduling tutor sessions. (3) I acknowledge that I have received a minimum grade of A in each of the aforementioned courses and have a CGPA of at least 3.0 (4) I agree to allow the Dean's Office to confirm my eligibility to tutor as per the guidelines. (5) Finally, I have read and understood, and I agree to, the "UMES Tutor Registry: Ethics Guidelines" found at http://umes.mb.ca/page/services/tutor-registry
