*Please Note: Each facility request requires separate application and application fee
If your event will serve food or will host 100 or more people please fill out the additional Large Event form prior to your request.
You may be required to have the following:
* Rental Fee
* Custodian as per CBA agreement ($40.00 per hour, 4 hour minimum)
* Kitchen worker ($25.00 per hour, 3 hour minimum)
* ALL food served must have either a SPS Kitchen worker or a Vendor permit with the Town of Sandwich Health Dept
* Police detail (Groups of 100 or more are required to notify both the Sandwich Police Department & the Sandwich Fire Department and fill out all necessary forms with them, 30 days in advance (indoors & outdoors) Failure to comply will result in denial of application. Fees paid directly to those departments
* Submit a liability of insurance certificate naming the "Town of Sandwich" as additional insurers. Coverage must include $1,000,000/$3,000,000 for bodily injury & property damage
