2023 PRACTIONER OF THE YEAR AWARD
Use this form to nominate an award winner for the 2023 PRACTITIONER OF THE YEAR. This award recognizes the outstanding efforts of a crime prevention practitioner for their contributions and outstanding performance in the field of crime prevention during the previous year.  The selected practitioner will be recognized at the 2023 Ohio Crime Prevention Association's Annual Conference Awards Luncheon on Tuesday, April 4th 2023.

NOMINATION CRITERIA:

-  The nomination can be made by any member of the agency employing the practitioner or by someone having
   personal knowledge of the practitioner's work.
-  Nomination should include a description of the outstanding work accomplishments of the individual  nominated.
-  Nomination should include the nominees current assignment, total years of service, and the community  impacted by their work.
