I understand that it is an honor and a privilege to apply for nomination in the national scholarship and fellowships competitions administered by the Office of National Scholarship Advisement (ONSA)

By signing below, I agree to conduct myself in a manner that complies with the University's Code of Student Conduct, as well as the behavioral expectations of award recipients, throughout the award competition. I acknowledge that honest and full communication during this process will benefit both me and the University, and I hereby commit myself to that level of communication.

Furthermore, pursuant to the Family Educational Rights and Privacy Act, commonly known as FERPA, I give permission to ONSA to request unofficial and official copies of my transcript(s), course grades, GPA and other applicable educational records for the purpose of the evaluation of my application. I understand that I can revoke this permission at any time.
 
I grant permission to ONSA and the University of Mississippi to publish in print, electronic, or video format my biographical information and record or edit into video and/or photographs my name, likeness, voice, image, and/or video images or image of myself to publicize my nomination and/or receipt of this scholarship or fellowship.  I also consent to have a copy of my application and all supporting materials retained by ONSA and understand that portions of my application may be made available to future applicants as an example for them to review as they prepare their own applications.

I certify that all of the information I include in my application for this award, including listings of activities and awards, research undertaken or planned, and personal statements or other essays, are my own work and are true and accurate to the best of my knowledge.

I understand that, should I have any questions, I may discuss the terms of this waiver with ONSA.

I hereby waive my right to view faculty and institutional letters of recommendation, endorsement, or evaluation which are written for the purpose of this award competition.  While copies of these letters may be provided to me by the authors, I understand that this is done as a courtesy by the author and does not affect this waiver.

Pursuant to FERPA, I hereby give permission for my grade point average (GPA), transcripts, individual course grades, and individual assignment grades to be reference, discussed, and/or otherwise used as a part of faculty and institutional letters of recommendation and endorsements and the applications for the purpose of this award competition.

I understand that institutional endorsement may be revoked at any time for reasons including but not limited to: inadequate progress on any component of the application; a change in academic status, standing, or decline in GPA; academic misconduct; engaging in behavior that does not align with the Code of Student Conduct or the behavioral expectations for award recipients.
