- Must live in Chicago Southside, Westside of Chicago or South Suburbs
- Child Must be ages 3yrs-18yrs Old
- Suffer from homelessness***
- Receive Public Aid Cash Assistance***
- Low- Mid Income-*** under 50,000 year

Media and Participation Agreement
My family and I agree to participate in City Motivators event and activities
We are also aware pictures are being taken throughout the event and they might be used to help bring awareness to CM mission programs, events and sponsorship needs as well as thanking all partners. Pictures may be used or promoted on all their social Media pages and website.
We agree to participate at our own risk, in all activities including the bounce house and
horseback riding (No Open Toe Shoes)
If no, please let a City Motivators team member know so they can bring your items outside of the event area.
Privacy: All information is kept private, but may be shared with resource and support partners.
