In case of minor injury, you permit us to administer minor first aid, i.e. cleaning wound, bandaids, ice pack.  In the event of a more serious injury we will attempt to contact parent and if needed emergency personnel.

I understand that there are NO in and out privileges for this event. If your student leaves the event, they will not be permitted back in. Please inform your student that they are expected to stay for the duration of the event. Chaperones will attempt to contact parents, but will not be responsible for any students that leave the event early.*

I understand that no bags of any kind will be admitted to the event. No purses, backpacks or other bags are allowed. Students may request a bag at check in if needed.

By signing my name below, my student and  I acknowledge all of the rules above and that we understand that all the same rules which apply while on campus at Marshall Middle school will apply while at the 8th grade party event itself.  We are also aware that there will be a zero-tolerance policy in place for any conduct outside of these rules/regulations. Anyone who chooses to conduct themselves in a manner that goes against these policies will be removed from the party and will be asked to sit with the parent volunteer/s in charge of security until a parent/guardian can be reached to take the student home.*
