Parish Group Registration Form
This group registration confirms that the appropriate leadership and chaperones will be present. It is important to note that by filling out this google form, your registration is not completed. 

Once this form is approved, a link will be sent to the PCL to forward to parents and chaperones allowing families and chaperones to register online and upload the appropriate documentation. Parishes will have access to this information so no further permission slips are needed unless the parish is providing transportation.

REMINDER: The number of chaperones is 2 for the first 8, and then 1 more for every 8 subsequent students.
