*Only fill this form out if there has been a CHANGE, OR YOU ARE NEW TO THE DISTRICT and have not filled out this form.
* Fill the form out once for each student who needs updated.
* Students will not be routed on a bus until this form is completed and returned.
* This information is used to determine your ONE scheduled pickup and ONE scheduled drop-off point
*Students will only be allowed to ride ONE bus.
* Complete this form for EACH student.
* Minimum of 48 hour notice (2 School Days) before changes are put in place.
