This form is used to communicate how students are getting home at the end of the day.  Afternoon transportation change request must be made the day of the change prior to 1:00 pm. ***A request must be submitted per student.

Absence: If student is absent, DO NOT fill this form out. Fill out absence form on the SAIL website or email attendance.sail@cms.k12.nc.us

Early Dismissal: Please do not fill this form out. Parents must notify the teacher. Early dismissal ends at 1:45pm.

Busing changes: For long term changes to bus routes, please refer to the CMS Transportation page.​
