*By registering, I agree to the terms below made by Sara J. O'shea and the retreat facility, Holiday Shores Resort.   Completed registration form and payment is required to reserve a spot at the Ladies' Getaway at the Lake.  Reservations are available on a first come/first served basis.  A registration form is required for each individual participant.  All participants must be 18 years of age or older.  Payment of registration fee is to be made to Sara J. O'shea in the form of cash or personal check.  Registration is non-refundable.  Registrations may be transferred until September 30th, 2014.  After this date, no transfers can be made and all fees paid are non-refundable.   Holiday Shores Resort cottages are non-smoking. For guest's convenience, smoking is allowed on the private deck located at each cottage. A smoking receptical is provided for guest convenience. Violation of the non-smoking policy will result in the guest being asked to vacate the resort and loss of monies paid in addition to a $200 fee.  The resort has a strict NO PET POLICY. Violation of the no pet policy will result in the guest being asked to vacate the resort and loss of monies paid in addition to a $200.00 fee.  Check can be mailed to Sara J.
