Request for Student Absence Form

Parent or Guardian Notification  (**Please read carefully before completing this form**).

As stated in the Student/Parent Handbook, “Parents who wish to take their student(s) out of school for more than two (2) days must complete a “Request for Student Absence” form at least seven (7) days prior to the scheduled absence. Before leaving, students are responsible to complete all class work which will be assigned during the days of their absence unless teachers have stated otherwise. Such special arrangements must be documented. It is to be understood that the days of absence are unexcused and are charged against the maximum ten (10) days for the semester. Such absences do not obligate the teacher to provide makeup tests or quizzes, nor to accept any overdue reports or assigned work.  

This form will now be completely electronic. Once you have answered the questions below, you will receive an email (via the email address you provide below) with a document attached. You will need to access this document in a few days to see if the principal has approved the request.
