If you need to register more than one attendee, please fill out this form for each attendee. Each registration will receive a separate invoice for $220 via PayPal, to be emailed to the person designated to receive the invoice in this registration form; the invoice can be paid immediately with a credit/debit card or with a check mailed to: Gifted Association of Missouri, PO Box 181, Saint Joseph, MO 64502. If your district requires an invoice to include a purchase order number on the invoice, please have that number ready before registering (and include in this form).
